Dr. Virus is a captivating falling block tile-matching video game that puts players in the role of Dr. Virus, a doctor tasked with strategically tossing two-colored medical capsules into a medicine bottle that serves as the playing field. The playing area is inhabited by viruses of three different colors: red, yellow, and blue, stationed in their starting positions until cleared by the player. The player must deftly manipulate each falling capsule, moving it horizontally and rotating it 90 degrees in either clockwise or counter-clockwise directions.

Aligning four or more capsule halves or viruses of the same color vertically or horizontally leads to their removal from play. Any remaining capsule halves or whole capsules that are no longer supported by a virus or capsule will descend to the bottom of the playing field or come to rest upon another supported object. Additionally, any new alignments of four or more in a row resulting from these falling capsules will also be eliminated.
